Steps Needed to Begin Service

  1. Contact your current insurance company

  2. Request a PCA Assessment (From Certified Public Health Nurse)

  3. Choose a Provider Agency

  4. Choose a PCA

If you have not contacted your insurance carrier to request a PCA assessment please do so, this is required before PCA services can begin.
